Steps to make an appointment

Find the provider you would like to schedule an appointment with
STEP 2
Select the day you would like to make an appointment
STEP 3
Select a time you would like to see that provider
STEP 4
Complete the required personal information
click the arrow on the right side of the appointment box to continue to the next step
STEP 5
Complete the required contact information
STEP 6
Press the "finish" button to submit request
Your appointment is not confirmed until you are notified by the clinic
